Treatment Specialties

Mental Health Targeted Case Management (MHTCM)

Targeted case management services are services to clients within targeted groups. The target population that may receive Mental Health Targeted Case Management (MHTCM) as part of the Texas Medicaid Program are clients, regardless of age, with a single diagnosis of chronic mental illness or a combination of chronic mental illnesses as defined in the latest edition of the American Psychiatric Association’s (APA), Diagnostic and Statistical Manual of Mental Health Disorders (DSM) and who have been determined via a uniform assessment process to be in need of MHTCM services.

Mental Health Rehabilitative Services

Mental health rehabilitative services are defined as providing assistance in maintaining or improving functioning and may be considered rehabilitative when necessary to help a client achieve a rehabilitation goal, as defined in the treatment plan. Mental health rehabilitative services may be provided to a client with a serious mental illness as defined in the latest edition of the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders (DSM).

Skills Training and Development

Skills training and development is training provided to an eligible client, the LAR, or primary caregiver on behalf of an eligible client. The training addresses: Serious mental illness and symptoms related problems that interfere with the client’s functioning and living, working, and learning environment Provides opportunities for the client to acquire and improve skills needed to function as appropriately and independently as possible in the community

Crises Intervention

Crisis intervention services are intensive community-based one to one service provided to clients who require services to control acute symptoms that place the client at immediate risk of hospitalization.

Medication Training and Support

Medication training and support services include education and guidance regarding medications and their potential side effects. These serve as a beginning orientation for the client in comprehending the nature of his/her emotional disturbances or mental illnesses and the significance of medications in assuring increased tenure in the community and symptom reduction.

Psychosocial Rehabilitative Services

Psychosocial Rehabilitative Services are social, behavioral, and cognitive intervention provided by members of a client’s therapeutic team that build on strengths and focus on restoring the client’s ability to develop and maintain social relationships, occupational or educational achievement, and other independent living skills that are affected by or the result of a serious mental illness in clients who are 17 years of age and older.

Supporting adults with serious mental illnesses

Find Your Destiny provides mental health rehabilitation services to adults aged 18 and older with severe mental illness or personality disorders that interfere considerably with their ability to perform primary aspects of daily living, such as self-care, interpersonal relationships, school, work, and household management.

Helping children and adolescents deal with behavioral or emotional challenges

At Find Your Destiny Counseling Services, we offer children and adolescents aged 6 through 17 mental health rehabilitation services to help them deal with behavior or emotional disorders that may restrict their performance in the areas of social, vocational, or personal skills as well as academics.
